Through Disease and Case Management, Cox HealthPlans’ Medical Management Team helps control healthcare expenditures while working diligently to ensure that our members receive the utmost excellence in care at the appropriate level.
Team members are here to help members and their families control out-of-pocket expenses and reduce the burden of navigating the healthcare system and health-plan policies.
Cox HealthPlans Case Managers work hand-in-hand with members, their physicians, and family to assist in managing complex conditions and to provide:
- Education to ensure a healthy quality of life
- Assure quality care based on clinical-care guidelines
- Coordinate care between physicians, specialists, and members
Our DISEASE MANAGEMENT PROGRAMS for diabetes, heart disease, asthma, and other chronic conditions are designed to help members take control of their health and improve their quality of life.

In an effort to help control costs, Cox HealthPlans has implemented various programs to assist members with their specific medical needs. A sampling of these Cost-Containment Initiatives are outlined below.
Research indicates the cost of care for back pain exceeds that of diabetes and even cardiovascular care!
Our BACK PAIN PROGRAM is designed (at no additional cost) to help members receive quality care and achieve quality of life through alternative-care measures that can be taken BEFORE resorting to costly spinal surgery—which is not always the best back-pain solution.
As an additional incentive, Cox HealthPlans provides qualified members with a prepaid debit card to pay for these alternative care measures!

Diabetes is a serious, lifelong disease and one of the most predominant health conditions in our country today affecting people of all ages and genders. It can lead to other more serious conditions if not properly monitored and regulated.
17.8 million people in the United States have been diagnosed with this disease, and approximately 5.7 million have not yet been diagnosed! Each year, 1.6 million people ages 20 and up are diagnosed with diabetes.
Cox HealthPlans’ DIABETIC SENSE PROGRAM is administered through CatalystRx, our pharmacy benefits administrator (at no additional cost). The program helps members with diabetes regulate this disease and maintain compliance with necessary medications.
Diabetic Sense has partnered with several companies to offer patients glucose test monitors, test strips, lancets, syringes, and alcohol pads at no cost. Among other benefits, patients receive the support of regular phone calls, letters, and brochures to remind them of needed refills and to emphasize the importance of regular blood-glucose monitoring.
