What is a Health Risk Assessment (HRA)?

A Health Risk Assessment or HRA is a screening tool that is used to help individuals identify potential health risks and to help understand their current health status. At Cox HealthPlans (CHP) our HRA is designed to connect members to wellness resources that will be helpful to them based on their responses. CHP also uses completed HRAs to help evaluate wellness and chronic condition programs.

The more HRA data CHP collects, the better CHP can tailor its programs to fit member needs.
